Please note that we require current contact details (including residential/postal address, contact numbers, email address) and original certified photo identification (such as a certified copy of a driver's licence or passport) for each applicant and any Third Party Authority. For further information regarding customer identification requirements please click on the attached BankWest Certfication Guidelines document. (This outlines the identification documents required for the opening of a BankWest Money Market account). If photo identification does not carry correct and current residential address details, please include verification of the correct address (such as bank statement, council rate or utility accounts including water, gas or electricity).

Broker to Broker Transfer Request Form

Use this form to transfer your investments from your previous stockbroker to us.

Issuer Sponsored Holdings to CHESS Sponsorship Conversion Form

Use this form to transfer your investments held with the original issuer to us. The term �Issuer� refers to shares that are not sponsored with a stockbroker, but by the Company issuing the shares.

Exchange Traded Options Client Agreement Forms

To activate options trading, you must have an existing trading account. You are required to read our ETO Product Disclosure Statement and the ASX Options Explanatory Booklet prior to trading options.

Spread Trading Kit

To activate spread trading, please complete all of these forms. If you are already an options client, you only need to complete the Auto Exercise Agreement Form. You are required to read our ETO Product Disclosure Statement and the ASX Options Explanatory Booklet and the ASX Options Strategies Booklet prior to trading options.

Warrant Client Agreement Form

To activate warrants trading, you must have an existing trading account. You are required to read the ASX Warrants Explanatory Booklet prior to trading warrants.
